Congratulations on being chosen to become a Godparent/Sponsor!
Here are the necessary steps:- Download the Godparent/Sponsor Certificate.
- Fill out the top with your Name, Address, Phone Number, Email and the Name of the Candidate
- Please carefully read the requirements to become a Godparent/Sponsor
- If you fulfill the requirements, please sign the document.
- If you do not fulfill the requirements, please notify the parents/candidate so that another Godparent/Sponsor can be chosen.
- Put the Certificate in an envelope marked – “Godparent/Sponsor Form to be Signed”
- Bring it to the church office (after hours leave it on the table outside the office).
- When the Certificate is signed by a Clergy member, you will be told to come by and pick it up.
- If there are any questions about eligibility, please speak to a priest/deacon.
